 Hispanics are underrepresented in clinical research making up only 1% of participants in clinical trials. Yet recent studies have shown that Hispanics are just as willing to  participate in health research as non-Hispanics so why is Hispanic recruitment such a challenge? The traditional school of thought has been to blame the issue on the language barrier. Yet even after your materials have been translated into Spanish, enrollment does not increase. The fact is that language is only a small piece of a bigger picture. Low literacy levels, ineffective communication by research staff; cultural beliefs and legal fears are all factors that play a much bigger part.  The good news is that these barriers can be overcome. First you must understand the different Hispanic segments and tailor your approach to reach the segments most appropriate for clinical research. Traditional recruitment methods are simply not effective with the foreign born Hispanic patient segment. Recruiting in the immigrant community is actually much less expensive than traditional recruiting but the messaging and venues must be changed. Relationship building and educational outreach is key but staff members must be culturally competent in order to impress rather than offend.  Recruiters can learn to bring Hispanics to your clinic but unless the environment is comfortable and the staff prepared, then you will not retain patients.
